Atestado do Medico Electronico. This is a document your doctor will fill out attesting that you are sane and of good health. It will include your vision status as well. This will vary by doctor and insurance.
Does it require a health card? Can't you go to a doctor and have it done without a Health Card? Just pay for the visit. And how would you find that doctor if you don't have one?
I had an appointment with the ACP for the visit but they wanted the health card. However, they didn't specify if this a general requirement or only for visits within the ACP.

I'm a little late for your answer, but it may help someone else.
Does it require a health card? - yes - you must be registered by the SNS and have the SNS number.
Can’t you go to a doctor and have it done without a Health Card? - No
Just pay for the visit. And how would you find that doctor if you don’t have one? - You can't. But my exam was done by my GP.
